Immunizations & Health Forms
Hawaii State Law requires all students to meet physical exam and immunization requirements before they may attend a public or private school in Hawaii. Students who do not complete the health requirements by the first day of school will not be allowed to attend school until they are met.
Health Requirements
- Tuberculosis (TB) Clearance: A Tuberculin (Mantoux) test within the last 12 months before the first day of school. (Note: This is only required for new students.)
- Physical Examination: A physical exam must be completed within 12 months before the first day of school.
- Immunizations: Complete series of immunizations based on age and interval requirements are required.
Beginning July 1, 2020, additional immunizations will be required for students entering 7th grade, and all students entering school in Hawaii for the first time, regardless of age. The updated immunization requirements conform with current national recommendations and reflect what occurs in healthcare provider offices and clinics in Hawaii as standard medical practice. New Students
All new students to La Pietra must have a TB clearance, annual physical exam and updated immunizations completed prior to the first day of school. Schedule an appointment with your doctor and ask your daughter’s physician to complete the Student Health Record Form 14 & TB assessment forms, which can be found in our Parent Resources. The Student Athletic Exam Forms are also required of all students, even if they are not participating in sports. This must be dated August 6, 2023 or later. Physical exams should be updated annually for sports participation.
Returning Students
All returning students must have a current health and immunization record in their student file. Please complete Student Health Record Form 14, which can be found in our Parent Resources. For student-athletes, a physical exam must be completed annually and within the last 12 months prior the beginning of the sports season. Simply ask your doctor to complete the Student Health Record Form 14 & Student Athletic Physical Exam forms which will meet the physician exam requirements for Athletes. These forms should be updated annually for sports participation.

A link to the Magnus Health portal can be found in parent/guardian My BackPack accounts, where parents will then upload the forms.
